How do I register to see an Action Provider?

  1. Click HERE to register as a new client and enter your demographic and insurance information.
     

  2. Look for an email from no-reply@therapyappointment.com asking you to click on a link to set up and manage your client profile (portal). You'll need to enter a unique username and password.
     

  3. Once in your client portal, please complete all of the online consent forms found in My Docs & Forms. 
     

  4. Send a HIPAA-compliant message through your client portal to Client Care with a copy of the front and back of your insurance card. 

    • If you have not received or cannot locate your insurance ID card, you can log into your insurance company's client portal and obtain a PDF of your insurance ID card.

    • If you are using your Employee Assistance Program (EAP) benefits, we still require a copy of your primary health insurance card should any change occur with your EAP benefits.  This also allows us to verify if you have any behavioral health benefits through your primary health insurance company.

We are scheduling in person and telemental health (video) appointments.
