1. Call your insurance company (the customer service number is listed on the back of your card) and ask these questions:

• Is Action Consulting and Therapy an in-network provider in my plan?

  • If yes...

    • What are my benefits for behavioral health appointments held in the office or by video (telemental health)?

    • Am I covered for couples or family therapy? If yes, what are those benefits?

    • What is my financial responsibility for each appointment? What is my copay, coinsurance, or deductible?

    • Are there any limitations to my behavioral health coverage?

    • Am I limited to the number of sessions allowed?
       

  • If no, Action Consulting and Therapy is not an in-network provider...

    • What are my out-of-network behavioral health benefits?

    • What is my financial responsibility for each of these out-of-network appointments?

    • Are there any limitations to my out-of-network coverage?

• If you are using your Employee Assistance Program (EAP) benefits, please provide us with the name and phone number of the EAP company, the authorization number, the number of authorized sessions, and start and end date for these sessions.
